Assessing with Facile Forms |

From the tool bar on the
Manage Forms screen click the New Icon
This will open the Add Form
Dialog.
It is important to
understand that Forms are made up of one or more “Pages”. Here
you are defining the properties that will apply to the form you are
creating. Use these values to define the Assessment form.

- Title -- Assessment
- Name -- Assessment
- Published -- Yes
- Width -- 500
- Height -- 400
- Preview -- Overlayed
- Log to DB -- All Values
- Email Type -- To Default
Address
Email Report -- All Values
Email XML -- No
- Description -- A basic
assessment application built using FacileForms 1.1.2 It can be loaded as a
package and then customized to contain as many questions and
scoring options as you choose.
There are two other tabs on
this screen that allow you to set scripts and pieces for the form. We
do not use these in this example so I will save that for another
tutorial.
Click the Save Icon to
return to the Form Manager
You will see your newly
created Assessment Form in the list of Forms that came with Facile
Forms. Notice in the pages column that there has been a page 1
created for you during the save process. This is where the fun
begins. A form in FacileForms can have as many pages as you need for
your task. At this point you should have some idea how many pages
your going to need and what you want them to do. In our example we
will have 4 pages.
Page 1 will be the
Introduction and Instructions for the user.
Page 2 will hold questions
Page 3 will hold questions
Page 4 will hold the score
and feedback fields as well as the submit button.
